Abstract

This FortaleceRas Project aims to ensure the integration of health action and service. The project had a significant and positive impact on the perception of health professionals, with the training and implementation of the project in line with care at the Basic Health Unit in Bela Vista, promoting promotion and prevention assistance in a continuous and resolute way for the health user . One of the main benefits arising from this project was the instruction of professionals, with the proposal to put the Framingham scale into practice as an experiment for classifying cardiovascular risk, which has simplified the management of demands in the unit. Furthermore, the provision of assistance to users by community health agents has shown a notable improvement, due to their greater ability to properly guide and refer users within the network. Another successful achievement has been the reestablishment of the activities of the hypertensive and obese group. The project guarantees support in the qualification of professionals, where it adapts a theme in line with care that works for the benefit of the population, and that is why we continue to work in synergy with the need for the FortaleceRas project. Although there are many perceptions and experiences within a single service, the contribution of the project within health service units is considered favorable. Thus, the permanence of this device within the scope of services is of great importance to continue the process of implementing the flow in the five lines of care that most require attention, investment and resources from the SUS. In view of the objective of the project, it was to implement, expand and train health professionals in a refreshing vision that puts into practice the instruments established by the project, aiming at a better quality of life for patients and in the intervention of health indicators.
